As the spinal cord descends from the brain, it travels through the spinal canal and nerve roots are sent off the main spinal cord through nerve root canals (neuroforamen) at multiple levels. Spinal Stenosis is a condition involving narrowing of the spinal canal and nerve root canal. This may result from bulging discs, degenerative disc disease, spur formation and facet joint arthritis. Generally, this condition develops gradually over time before symptoms are experienced. Imaging studies (X-ray, CT scan, MRI) will usually show narrowing of the spinal canal and nerve root canal.

Symptoms

Patients will experience buttock pain with possible low back pain, with the pain usually radiating down one or both legs. There also may be associated weakening of the legs or a tired feeling in the legs. Symptoms are usually exacerbated with walking or standing for any period of time and will generally improve after sitting down or leaning on something. Usually individuals use a walker or shopping cart to allow them to walk for longer distances with decreased pain.

Treatment

Spinal Stenosis can be treated with multiple modalities. Conservative treatment with physical therapy, medication, epidural steroid injections and selective nerve root blocks generally provide significant pain relief. If symptoms persist, spinal cord stimulation may be an effective option, or surgical decompression may be considered.
